For use with reporting receipt of cash or checks. The Foundation needs to know
who gave the money and for what purpose. These receipts could be in the form
of donations, entry fees, event sponsorship, etc. The form or format is not
mandatory, but the information is very important.
Do you have a great project for which you need funding? Just complete the
attached application to request approval.
Need a check sent out to reimbusre a vendor? Do you need to be reimbursed
for expenses incurred for an event? We will need the attached. Copies of
vendor envoices or expense receips are required.
